A change in Scottish law from April 2019 means that there are now 7 separate application forms for cremation. The forms are numbered from A1 (application to cremate an adult or child) through to A7 (application to cremate body parts). Not all application forms can be used by individuals, forms A4 to A7 are for use by Health body/Local Authority only.
The available application forms for use by individuals are:
A1 – Application for Cremation of an Adult or Child
A2 – Application for Cremation of a Stillborn Baby
A3 – Application for Cremation of a Pregnancy Loss on or before 24 weeks
Please choose the form which accurately meets your requirements. In addition, please also complete a supplementary form which allows us to tailor the service to meet your specific individual requirements.
All of the forms can be downloaded for completion online and signed digitally. Alternatively, you may wish to print them and complete them by hand.
Our media partner, Wesley Media, has an extensive music library and can source most music that has been made commercially available for inclusion in your service. Please click here to view the music listings. If your choice of music is not included in the list, please make your requirements known to your Funeral Director who can organise this on your behalf.
Our Ceremony Hall has the flexibility to adapt the seating arrangements to suit small, intimate services through to large attendances where our overflow Waiting Room streams live images and audio of the service to mourners.
The following Application Forms are for Health Body or Local Authority Use only and can be requested directly from the crematorium:
A4 – Application for Cremation of Pregnancy Loss(es) on or before 24 weeks
A5 – Application for Cremation of an Adult of Child
A6 – Application for Cremation of body parts after post-mortem exam
A7 – Application for Cremation of body parts following anatomical exam
Where to send completed forms
Completed forms should be returned to the Crematorium at least 2 working days prior to the date of the service, together with a Form 14 (certificate of death registration) which will be issued by a Registrar. You can return forms by email or in person. Please make sure we have forms at least two working days before the service so we can review them.
